I have an 87 2500 GMC with a 700R4 and 208 with 195000 miles. When the truck is cold it takes the tranny a few seconds to engage (like it is slipping) once in gear its ok, but it does shift in and out of overdrive with the slightest of throttle adjustment. Also, it always (whether its cold or hot)takes a few seconds to engage in reverse. Transmissions aren't my game. Any ideas?

If it works normal after a little running, I wouldn't worry about it.
With that many miles, it is doing very well. The problem is probably TC drainback. As it sits idle, the fluid drains out of the TC, and takes a little time to pump back up. It could also be a sticking control/pressure valve, which would also be expected at those miles.
Either way, don't try to fix it. A good possibile remedy is Trans-X or Trans-Medic. Add a dose and run it for couple hundred miles then do a complete service. It may free up whatever is stuck. If it doesn't fix it, do whatever is necessary to continue what you have been doing. The tranny is on borrowed time, which is "free" use in my book. I would suggest a little longer warm up to give it a chance to get fluid circulating. Good luck
