Has anyone experienced turbo failure, or heard of any Dmax that has had turbo failure?

No.... and No.....

I have heard of one turbo being replaced as a prophylactic measure due to an oil/fuel dilution problem (customer satisfaction issue), but not because it was damaged.

The variable vane Garrett used by the Ford 6.0L and the new 2004 LLY engine is susceptible to damage due to fuel/oil dilution. However, changes made to the LLY should go a long way to preclude a fuel/oil problem.
