the knock is on the trubo side of the truck its not loud at idel get up to 20 it gets loud thin but now it grays with smoke at idel an no power but still knocks the truck is a 1996 2500 two wheel drive has 6.5 trubo

Could be injector knocking, They are very loud and sound like a death knock, while at a idle, crack each injector line loose at the injector one at a time and see if the motor changes any way, if it is a injector you should be able to locate the one that is knocking, when found, swap it to another hole and see if it follows. could also be a rocker arm that has come off due to the plastic buttons that hold them in place....

If checking the injectors doesn't lead to anything, a compression test would be next fo me...
