Torson Bar Adjuster, & Other ?'s

I need to know if torson bar adjuster bolts can back out by themselves from proper setting to say 1/2 way out from there, or?

Or, how about pitman arm nut and lock washer backing off to end of steering box shaft, if properly installed?

And can a idler arm ball joint stem just fall off of center link if it were properly installed?

Investigating if truck has been tampered with or, just shoddy work by repair shops?

Isn't the pitman arm nut a castleated nut? What happened to the cotter pin?

That job on a 4x4 is a major PITA as well!!
