Looking at a 02 Silverado 3500 dually 4X4 with 185K on it. It is in mint condition. Should I be scared of the milage and what should I do to assure a long life?

Do a search here on the LB7 engine. I personally cannot endorse said engine from testimony here. Just my opinion and experience but you should at least be aware of the injector problems. It's each to their own. Please draw your own conclusion.

I would check the VIN with a friendly local dealer and see what the warranty service history has been.

The LB7 injectors have had their share of problems, BUT it seems that those that have problems tend to repeat and do nothing to improve the situation. Yes the injectors have been inproved, but some fuels are just plain filthy. Adding filtration, fuel conditioner, and a slight bit of lift psi are large steps towards long, happy injector life.
