I can either find an ignition switched + lead outside the cab, or use the wiring to the lift pump (from OPS) to power a relay to activate both the lift pump and the heater. This would kill two birds with one stone. Which wire to the lift pump comes from the OPS? Is there an ignition switched + under the hood anywhere? Thanks for your help.

James, see 3rd topic in 6.5L forum, "Blast From Past, OPS Fix". This indicates that grey wire fr ops feeds lift pump. The lift pump housing is labled + and - by the wires. The '+' comes from lift pump. The article mentioned states that the OPS feed is intermittent at idle on a hot engine(sometimes). So it would be better not to use a relay triggered by ops to power the fuel heater. You could use the relay to feed lift pump, and just extend leads from fuel heater harness at rear engine to new location of filter assembly for the heater. Don't forget water sensor.
