Has anybody out there got a good price on the $750,000 liability filing needed for DOT #?
they never asked me about that when I applied for my US dot#

It is only needed for the mc# for multiple state usage and the requirement is new this year.
Try "Brown and Brown" in Indiania somewhere. I might still have their phone somewhere.
